Should we be afraid of managed care? A theoretical assessment.

Managed care capitation contracts provide monetary incentives for doctors to save medical costs while standard health insurance contracts do not. The paper proposes an alternative model for insurance markets which is used to analyze managed care contracts. In our model, households would like to buy insurance for the possible need of a service. The distinctive aspect of our model is that providers of service have privileged information on the most appropriate procedure to be followed. In the managed care application of the model, doctors are the providers of the service and through a diagnosis have better information of the patient's health condition. Equilibrium in our model is always constrained eÆcient. A partial capitation contract arises when both the cost and net benefits of treatment are high enough. We show that a capitation contract provides incentives for doctors: i) to care about the likelihood households will obtain the good state of nature (altruistic behavior); and ii) to save medical costs (managed care behavior). Doctors, in this case, choose less medically eÆcient treatments as they would choose under a standard health insurance contract. Besides this, household' welfare is increased in comparison to the standard contract. This increased welfare translates into a revealed preference for the capitation contract.

Fault diagnosis based on identified discrete-event models

International audienceFault diagnosis of Discrete-Event Systems consists of detecting and isolating the occurrence of faults within a bounded number of event occurrences. Recently, a new model for discrete-event system identification with the aim of fault detection, called Deterministic Automaton with Outputs and Conditional Transitions (DAOCT), has been proposed in the literature. The model is computed from observed fault-free paths, and represents the fault-free system behavior. In order to obtain compact models, loops are introduced in the model, which implies that sequences that are not observed can be generated leading to an exceeding language. This exceeding language is associated with possible non-detectable faults, and must be reduced in order to use the model for fault detection. After detecting the fault occurrence, its isolation is carried out by analyzing residuals. In this paper, we present a fault diagnosis scheme based on the DAOCT model. We show that the proposed fault diagnosis scheme is more efficient than other approaches proposed in the literature, in the sense that the exceeding language can be drastically reduced, reducing the number of non-detectable fault occurrences, and, in some cases, reducing also the delay for fault diagnosis. A practical example, consisting of a plant simulated by using a 3D simulation software controlled by a Programmable Logic Controller, is used to illustrate the results of the paper

This paper aims to raise the theoretical discussion about the vicissitudes in the process of teenage girls becoming women, from the psychoanalytical point, which says the puberty is a landmark on sexual development. Adolescence is approached as a sexuation process resulting from Oedipus. For the boys, the identification with the father is enough, while the girls need extra work. We are going to examine the proper impasses and the outputs to the femininity construction incented by Wendla and Ilse, adolescent characters from the play Spring Wakening by Frank Wedekind, in the face of the real drive.

It aims to identify on the websites of university libraries the practice of education of users through products and services that can be fostered in both traditional and digital/virtual environments with a view to promoting students' information literacy. Research classified as exploratory, established in its drawing two universities: the Universidade Federal de Alagoas and the Universidade Federal de Minas Gerais. Data analysis was performed with a quantitative and qualitative approach. It was adopted as a collection instrument the form, which provided the data collection in 35 libraries of the two institutions. The results provided evidence that the libraries of the two universities are in transition processes when considering the use of digital technology viewpoint to enhance user' education actions to develop information literacy
